1Name of person/organization seeking support and contact details including mobile phone, email address, mailing address.

2Details of the project that you are suggesting.

3Details of the children involved and the community and how they both will benefit.

4The number of children likely to benefit.

5What would be the benefit of our support and the likely legacy of that support?

6Any additional information you feel is relevant.

This doesn't have to be an overly detailed or complicated application. Give us the facts and the benefits as you see them.
